There are two versions in this document with different calls to action.

- The second is designed to accompany the client assessment questionnaire. This version is for a client that may or may not be a good fit. They can complete the information and return it to you so you can assess their needs.
Download this document, customize it with your practice name, and tweak the wording to match your personal style. Remember to include a call to action and let them know how you will follow up.
